Understanding Our Service Agreement

These terms govern your use of DAYASA Health and Wellness services and establish our commitment to providing exceptional regenerative medicine care.

Service Agreement

These Terms and Conditions govern your use of DAYASA Health and Wellness services and establish the legal relationship between you and our practice. By scheduling an appointment or receiving services, you agree to these terms.

Effective Date: June 16, 2025 | Last Updated: August 25, 2025

1. Acceptance of Terms

By accessing our website, scheduling appointments, or receiving services from DAYASA Health and Wellness, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, please do not use our services.

2. Medical Services and Treatment

DAYASA Health and Wellness provides regenerative medicine treatments and wellness services. All treatments are provided based on current medical standards and evidence-based practices. We specialize in non-surgical regenerative therapies including but not limited to:

  • Joint and mobility care treatments
  • Autoimmune disease support therapies
  • Cardiovascular regenerative treatments
  • Neural regeneration therapies
  • Skin rejuvenation procedures
  • Hair restoration treatments
  • Autism care programs

Treatment outcomes may vary based on individual health conditions, adherence to treatment protocols, and other factors. We do not guarantee specific results from any treatment or procedure.

3. Patient Responsibilities

As a patient, you agree to:

  • Provide accurate and complete medical history and health information
  • Disclose all current medications, supplements, and treatments
  • Follow prescribed treatment plans and post-treatment care instructions
  • Attend scheduled appointments or provide adequate notice for cancellations
  • Communicate any changes in your health status or concerns promptly
  • Maintain respectful communication with our staff and healthcare providers

4. Appointment Scheduling and Cancellation Policy

Appointments must be scheduled in advance through our designated booking system or by contacting our office directly. We require 24-hour advance notice for appointment cancellations or rescheduling. Failure to provide adequate notice may result in cancellation fees.

No-show appointments (failure to attend without prior notice) will be charged the full service fee. We understand that emergencies occur, and we will work with patients on a case-by-case basis for legitimate emergency situations.

5. Payment Terms and Financial Responsibility

Payment for services is due at the time of treatment unless alternative arrangements have been made in advance. We accept cash, major credit cards, and approved financing options. Patients are responsible for understanding their insurance benefits and coverage limitations.

Many regenerative medicine treatments are considered elective and may not be covered by insurance. We will provide cost estimates prior to treatment, but patients are ultimately responsible for all charges incurred.

6. Informed Consent and Treatment Risks

All medical treatments carry inherent risks and potential side effects. Before beginning any treatment, patients will receive detailed information about the proposed therapy, including potential benefits, risks, and alternative treatment options. Informed consent will be obtained prior to all procedures.

Regenerative medicine is an evolving field, and while treatments are based on current scientific evidence, long-term outcomes may not be fully established for all therapies. Patients acknowledge this uncertainty and consent to treatment based on available information.

7. Privacy and Confidentiality

We are committed to protecting your privacy and maintaining the confidentiality of your health information in accordance with HIPAA and other applicable privacy laws. Please refer to our Privacy Notice for detailed information about how we collect, use, and protect your personal health information.

8. Limitation of Liability

DAYASA Health and Wellness provides medical services in accordance with current standards of care. While we strive for optimal outcomes, we cannot guarantee specific results from any treatment or procedure. Our liability is limited to the direct cost of services provided.

We are not responsible for indirect, consequential, or punitive damages. Patients acknowledge that regenerative medicine involves inherent uncertainties and consent to treatment understanding these limitations.

9. Intellectual Property

All treatment protocols, educational materials, and proprietary information remain the intellectual property of DAYASA Health and Wellness. Patients agree not to reproduce, distribute, or share confidential treatment information without express written consent.

10. Governing Law and Dispute Resolution

These terms are governed by the laws of the State of California. Any disputes arising from these terms or the provision of services will be resolved through binding arbitration in Los Angeles County, California, in accordance with the rules of the American Arbitration Association.

Patients retain the right to file complaints with appropriate medical boards and regulatory agencies. We encourage open communication to resolve any concerns before formal dispute resolution becomes necessary.

11. Changes to Terms

We reserve the right to modify these Terms and Conditions at any time. Changes will be effective immediately upon posting on our website. Continued use of our services after changes are posted constitutes acceptance of the modified terms.
